GWC Community Services provides Home Care Packages to support older Australians with higher care needs to remain living safely and independently at home.
A Home Care Package can provide a coordinated range of services tailored to your individual needs, goals and preferences. Support may include personal care, domestic assistance, meals, transport, social support, nursing, allied health and other clinical services.
Our experienced team can help you understand your package, navigate the process and access services that respect your language, culture, faith and family needs.

Getting Started with a Home Care Package
- Register with My Aged Care.
- Complete an ACAT assessment.
- Receive approval for a Home Care Package.
- Wait for your package to be assigned.
- Choose GWC Community Services as your provider.
Home Care Packages are funded by the Australian Government and accessed through My Aged Care.
